Not all providers participate in every plan. If you are not yet enrolled in an EyeMed plan, please keep in mind your actual network may vary from what appears on our locator.

EyeMed members should log in to access the exact list of available providers. Even if a provider is displayed on this site, you should call ahead to confirm the provider still accepts your EyeMed plan. While walk-in appointments are available at many locations, you can also contact the provider to schedule an appointment.
